evolved girl® Program Consent, Terms & Conditions Who Can Enroll? - If you are 13 or older, you may enroll yourself in this program. - If you are under 13, a parent or legal guardian must complete enrollment for you. Privacy & Safety - Your information is private and will not be shared with your parent/guardian unless there is a serious safety concern—such as risk of harm to yourself or others—or if required by law. - We do not contact your parent/guardian about your participation or progress unless you ask us to, or unless there is a threat to your safety or the safety of others. Program Purpose - This program is for educational and informational purposes only. It is not a substitute for therapy, counseling, medical advice, or licensed mental health services. Supervision & Responsibility - If you are a parent or guardian enrolling a participant under 13, you agree to supervise your child’s use of the platform and accept responsibility for their online conduct. - All participants are expected to engage respectfully with instructors and other students. Bullying, harassment, inappropriate language, or disruptive behavior will not be tolerated and may result in removal from the program. Sensitive Topics, Support & Mandated Reporter Notice - Some discussions may involve personal experiences or emotional topics. If you share concerns related to mental health, safety, or serious personal issues, we may need to notify your parent/guardian or appropriate authorities only if there is a threat to harm yourself or others, or as required by law, to ensure your safety and well-being. - Mandated Reporter Notice: As a certified school counselor, I am a mandated reporter. This means that if you share information suggesting abuse, neglect, or a serious risk of harm to yourself or others, I am legally required to report it to the appropriate authorities. Faith-Based Program Notice - Some programs may include faith-based teachings and biblical principles. Participation is voluntary, and you/your parent or guardian are encouraged to review program descriptions to determine if the content aligns with your family’s values. Media & Testimonial Consent - By enrolling, you give permission for evolved girl®, Evolved You, LLC, and Dr. Jacqueline Deas to use testimonials or feedback you voluntarily provide for educational or promotional purposes. - Only your first name or initials will be used—never your full name or any information that could identify you. - No photos, videos, or other personal identifying information will be shared without your (or your parent/guardian’s) explicit permission. - If you do not want your feedback or experiences shared, please notify us in writing.
