By proceeding with enrollment, you confirm that: • You are the parent or legal guardian of the enrolled participant. • You consent to your child’s participation in this educational program. • You understand that this program is provided for educational and informational purposes only and is not a substitute for therapy, counseling, medical advice, or licensed mental health services. • You agree to supervise your child’s use of the platform and community features and accept responsibility for their online conduct. • You acknowledge that no specific outcomes are guaranteed, and participation is voluntary and at your own discretion. • You understand that your email and your child’s information may be used to communicate program updates, resources, and relevant information from evolved girl® and Evolved You, LLC. • You understand that information is handled responsibly and in accordance with applicable privacy laws, including COPPA (Children’s Online Privacy Protection Act) and GDPR where applicable. Children under the age of 13 may not participate without verifiable parental consent in accordance with COPPA regulations. For participants located in the European Union, parental consent requirements comply with GDPR regulations for children under 16 (or the applicable age under local laws). Faith-Based Program Notice Some programs offered through evolved girl® and Evolved You, LLC may incorporate faith-based teachings and biblical principles as part of their personal growth and character development framework. For example, programs such as evolved girl Academy include faith-based concepts designed to encourage spiritual growth, character development, and purpose-driven living. Participation in these programs is voluntary, and parents are encouraged to review program descriptions to determine if the content aligns with their family’s values. Some programs may include group mentoring or coaching components. Community Participation & Conduct Some evolved girl® programs may include online communities, live sessions, or interactive discussions. Parents acknowledge that they are responsible for supervising their child’s participation and online behavior within these environments. Participants are expected to engage respectfully with instructors and other students. Bullying, harassment, inappropriate language, or disruptive behavior will not be tolerated. Evolved Girl® programs are designed to maintain a safe, respectful, and encouraging environment for all participants. Any violation of these expectations may result in removal from the program or community without refund. Sensitive Topics & Participant Support Some discussions during group mentoring, coaching sessions, or community interactions may involve personal experiences or emotional topics related to growing up, relationships, or personal challenges. While evolved girl® programs are designed to provide encouragement, guidance, and personal development support, they are not intended to provide counseling, therapy, or clinical mental health services. If a participant shares concerns related to mental health, safety, or serious personal issues, parents or guardians may be notified and are encouraged to seek support from qualified licensed professionals when appropriate. Parents and guardians remain responsible for their child’s mental health care, well-being, and professional support needs. Media & Testimonial Consent (Optional) From time to time, evolved girl® and Evolved You, LLC may share general program highlights, testimonials, or written feedback from participants and families for educational or promotional purposes. By enrolling in the program, you grant permission for Dr. Jacqueline Deas, evolved girl®, and Evolved You, LLC to use testimonials or feedback voluntarily provided by participants or parents in program materials, websites, or promotional content. No photos, videos, or personal identifying information about minors will be publicly shared without parental awareness and consent. Parents who prefer that their child’s feedback or experiences not be shared may notify the organization in writing. Liability Clause By enrolling in and participating in this program, you acknowledge and agree that Dr. Jacqueline Deas, Evolved You, LLC, evolved girl®, and any associated team members, affiliates, or representatives shall not be held responsible or liable for any decisions, actions, or outcomes resulting from participation in this program. All activities, content, and guidance are provided in good faith and are intended for educational purposes. Participants and parents are responsible for how they choose to apply the information provided. By participating, you agree to hold harmless and release from liability Dr. Jacqueline Deas, Evolved You, LLC, evolved girl® programs, and all affiliated individuals or entities from any claims, demands, damages, or liabilities, including but not limited to direct, indirect, incidental, or consequential losses arising from participation in the program.
